The role of a referrals and admissions coordinator is crucial in managing the initial enquiries and referrals process at STEPS Rehabilitation and involves assessing, and gathering comprehensive information about clients, their conditions, and their rehabilitation needs.
When individuals first contact us, they often have numerous crucial questions regarding how STEPS can help them and around the funding options for their rehabilitation. The referrals and admissions coordinator will discuss the range of therapies, medical support and care options available and arrange visits to allow potential clients and their families the opportunity to see firsthand what makes STEPS so unique.
As part of our dynamic team, the referrals and admissions coordinator will work alongside the referrals and admissions lead, supporting and coordinating the entire process from initial contact to admission.
This is a fast-paced role that requires excellent problem-solving, prioritising, and clinical reasoning skills.
Why This Role Matters
For individuals who have experienced life-changing injuries or illnesses, the journey to recovery can be daunting. The Referrals and Admissions Coordinator plays a pivotal role in easing this journey, providing essential support and guidance from the first point of contact.
Your Key Responsibilities will include:
- You will be the first point of contact for initial enquiries from potential clients and their families, addressing their questions about the rehabilitation process and discussing funding options.
- You will conduct thorough pre-admission assessments to gather detailed medical information and understand the client's goals, hobbies, and preferences. These assessments can be conducted in person or virtually where needed.
- You will collaborate with the interdisciplinary team to create an individualised rehabilitation proposal including cost estimates and anticipated length of stay.
- You will ensure clients receive all necessary information prior to admission, including a welcome pack and visitor arrangements.
- You will ensure that each client’s journey is as smooth and supportive as possible as they transition into STEPS and be a continued point of contact for them throughout their stay as needed.
